A newborn baby’s death was determined to have been “potentially preventable” because of the “abnormal heart rate” that went unnoticed for 26 minutes. Unfortunately, Bodhi Leo Searle did not make it past his first day of life after being born on August 30, 2021 at Flinders Medical Centre in south Adelaide.
Coroner Naomi Kereru is leading the investigation into Bodhi’s death which includes the time his mother Diana Searle’s heart rate was mistakenly monitored instead of his.
What Happened With Mrs Searle on August 29, 2021?
According to the Adelaide Advertiser, counsel assisting the coroner Sally Giles testified that an expert witness will testify that Bodhi’s death was “potentially preventable” if his heart rate was monitored correctly. Mrs Searle, Bodhi’s mother had a healthy pregnancy and went into labour on her 30th birthday, August 29, 2021.
She arrived at the hospital around 5:30 p.m. Mrs Searle was informed by her regular midwife that she was ill and she was transferred to the care of Stephanie Geyer and first-year student midwife Thea Koke.
The inquiry was informed by Ms Giles that Ms Geyer first noticed something was wrong with Bodhi’s heart rate at 11:26 p.m.
As per Msn reports, At 11:44 p.m., Mrs Searle was rushed to the hospital where she was placed on CTG monitoring. But at about 12.15 am, it was discovered that the CTG trace had been recording the mother’s heart rate instead of the foetal one for about 30 minutes.
Once the severely abnormal foetal heart rate was detected, the necessary corrections were made. The ward’s registrar and sole obstetrician expressed reluctance to attempt a “complex instrumental delivery” alone. Bodhi was, however, born naturally at around midnight.
The decline in Bodhi’s health, according to Ms Giles was immediately apparent. His Apgar score (which evaluates factors such as a baby’s colour, heart rate, reflexes, muscle tone and breathing) was a pathetic zero and he looked it.
‘There was no evidence of any respiratory effort and there was no identifiable heart rate or pulse present,‘ Ms Giles said. For 18 minutes, Bodhi lay in a coma-like state without breathing. Doctors in the neonatal intensive care unit discovered signs of abnormal brain activity and rushed him there.

What Happened to Newborn Baby Bodhi Searle?
On August 31, 2021, at 1:18 p.m., Bodhi Searle left this world. An examination of his brain tissue revealed hypoxic ischaemic encephalopathy, an injury to the brain brought on by a lack of oxygen. She testified that Associate Professor Stefan Kane found Bodhi’s death was “potentially preventable,” as Ms Giles put it.
Ms Giles speculated that if the CTG had been used sooner and if the foetal heart rate had been confirmed earlier rather than the maternal heart rate, it would have been possible to detect the foetal compromise and prompt efforts to speed up the birth.
Thea Koke, a student midwife gave the first testimony at the inquest. She claimed responsibility for affixing the CTG monitor to Mrs Searle and mentioned that Ms Geyer made some adjustments to the device. Ms Koke stated that she had placed Bodhi on Mrs Searle’s chest prior to the start of resuscitation efforts because there had been “no extreme urgency” in the delivery room.
It was Bodhi’s appearance, she said, that caused immediate alarm. Southern Adelaide Local Health Network of which Flinders Medical Centre is a part issued an apology for the “tragic circumstances surrounding” Bodhi’s death through Lauren Gavranich.
