Phoenix was only 23 when he died on Halloween morning, 1993 from an overdose of cocaine and heroin a mixture commonly referred to as speedball at The Viper Room in West Hollywood. Phoenix had flown back to Los Angeles for a week in New Mexico in late October 1993. Previous to that, he had spent about a month and a half in Utah finishing up the remaining three weeks of interior shots on his previous film, Dark Blood. In 2012, production wrapped up on the film.

Forrest went outside after hearing a commotion inside the club and saw Mathis screaming while Phoenix lay on the sidewalk and shaking violently. Joaquin dialed 911 because he couldn’t tell if Phoenix was alive or not. Once Phoenix had stopped breathing, Rain began performing mouth-to-mouth resuscitation on him. As the band played “Michael Stipe” Gibby Haynes claims that Phoenix was having a seizure outside on the sidewalk.

As word of the incident spread through the nightclub, Flea abruptly left the performance. Paramedics had arrived by that time and discovered a cyanosing Phoenix in the midst of cardiac arrest and asystole. They gave him medication to try to get his heart beating again.

Phoenix was still alive when the ambulance arrived, so Flea went with him to Cedars-Sinai. The medical staff at the hospital were unable to revive Phoenix despite their best efforts. At 1:51 a.m. PST on October 31, 1993 his death was confirmed.
