Directions for using the Digital Passenger Locator Form for passengers

Public health agencies utilize Passenger Locator Forms (PLFs) to allow cross-border contact tracing if travelers are exposed to an infectious illness when traveling by plane, ship (cruise/ferry), train, bus, or vehicle.

Public health authorities in the departure, transit, and destination countries can use the information provided in PLFs to rapidly contact travelers and other close contacts of an infected person to protect their health and the health of their contacts and prevent further disease spread.

An online application for a European Digital Passenger Locator Form (dPLF) was created in response to the COVID-19 pandemic to ease the use of PLFs during cross-border health concerns.

This digital format will make data gathering and transmission between stakeholders simpler and more practical, hence improving the effectiveness and efficiency of contact tracing.

Future visitors entering a nation through the air, marine (cruise or ferry), or ground (train, bus, vehicle) transit may be required to submit a digital Passenger Locator Form while utilizing the European dPLF online service.

How To Apply?

Beginning on May 24, 2021, everybody who enters Italy (as their final destination) from a foreign state or territory on any mode of transportation before entering the national territory will be obliged to complete the digital Passenger Locator Form.

The traveler will get the dPLF in pdf format and a QR code through email to the address provided at registration after the form has been received. The form must be presented straight from your smartphone (in digital format) at boarding and to whoever is designated to conduct controls.

The traveler may also print a copy of the dPLF to provide at boarding. The dPLF must be filled out before boarding. The field about the assigned seat number for the mode of transportation will always be changeable.

Each adult passenger must complete a form; if there are kids present, they can be registered on the form of an accompanying adult. Before departure, the dPLF must be completed by the guardian of unaccompanied kids.

Passengers traveling to one of the countries listed below (as their final destination) are required to fill out a passenger locator form using the European Digital Passenger Locator Form (dPLF)

Each nation has its own rules on who is required to finish the PLF (some countries choose if a family is traveling together, only one family member must complete the PLF and other countries require that each adult person completes an individual PLF).
